Please complete either of the following forms in order to be included in the Supplementary Register of Electors;

Completed forms to be submitted to Waterford City & County Council, Register of Electors, City Hall, The Mall, Waterford or by hand before 4 pm to The Customer Services’ Desk in Bailey’s New Street, Waterford or County Hall, Dungarvan.
